Please arrive 15 minutes before your scheduled appointment, and be sure to download, complete and bring the below forms with you. If you have any questions about your upcoming visit, please call us. We’re here to help.

We encourage everyone to be on time to allow all our patients to get in and out of our office with ease.  To help keep this timely schedule, we do have a reservation policy of calling the office 48 hours or more in advance of your existing appointment to avoid a cancellation fee.  

We do realize that unexpected events do happen, so we do take those into account.  If you find you cannot keep your appointed time, please be sure to call our office at your earliest convenience.  If we cannot answer, please leave a voicemail on our answering machine, and we will be sure to check it the following business day.
